This is with reference to below mentioned child who has heart defect by birth, which needs surgical intervention.
Name : Ms. Arohi Sitendra Panda
Age : 2 years 05 months old (DOB – 20/10/2022)
Sex : Female
Diagnosis : Dextrocardia, DORV, VSD, PS, Hypoplastic LV
Surgery : BD Glenn Shunt with Atrial Septectomy
Estimate : Approx Rs. 1,95,000/-

Ms. Arohi Sitendra Panda is having the above disease by birth. After birth, the patient was
having bit of discomfort and weakness, as a result of which her parents consulted with doctor
at Mulund Mumbai, this patient referred to Pediatric Cardiac Dept Fortis Hospital Mumbai.
And child confirmed with Heart defect and suggested for immediate treatment. As soon as they
receive the trust approval we will plan for surgery.

Mr. Sitendra Simanchal Panda is father of the child. He is currently residing at Indira Nagar
No. 2, J N Road Mulund West Mumbai – 400080. He is a “Factory Worker” & his annual
income is Rs. 70,000/- His wife is a house wife. Ms. Arohi is their younger daughter who is
suffering from the above-mentioned disease.

There is no other earning member in the family & he does not have any savings and absolutely
is not in a position to meet the cost of the treatment. Mr. Sitendra is finding it very difficult
to manage the funds for his child’s surgery. Therefore, I would like to request you to consider
his application and help his family financially for the surgery and support them in this time of
distress

Baby Arohi Panda’s surgery was successful and baby has been shifted from ICU ward to general ward. Baby would require 1 week of hospital stay.
